of our services. There is a possibility that the information contained in e-mails can be accessed by third
parties and the Clinic cannot guarantee the security of the information transferred from your e-mails

Jurisdiction

Antalya Courts and enforcement offices are authorized for disputes arising from these terms and
conditions of use agreement or the use of the website and its sub-pages.

GENERAL DESCRIPTIONS

Personal data owners (“Relevant Person”) defined as relevant persons in the Personal Data Protection
Law No. 6698 (“Law”) can exercise the rights granted in Article 11 of the Law regarding their personal
data and must apply to the Data Controller to exercise these rights.

In accordance with the 1st paragraph of Article 13 of the Law; Applications regarding these rights to
the Land Of Smile Clinic (Swident Private Sağlık Hizmetleri A.Ş.) (“Company”), as the data controller,
must be made in writing, via e-mail on the internet or in other ways determined by the Personal Data
Protection Board (“Board”).

Applications submitted will be answered as soon as possible, within thirty (30) days at the latest,
depending on the nature of the request. Requests are free of charge unless exceeding limits defined
in the Communiqué (e.g. >10 pages or media costs).
